The Home Care Teams at Delaware County Memorial Hospital (DCMH) and Taylor Hospital were named in the 2008 HomeCare Elite, a compilation of the most successful Medicare-certified home health care providers in the United States – DCMH for the third year and Taylor for the second year.

 

“A hospital-based home health care program may shorten or eliminate the need for an inpatient hospital stay, help the patient delay or avoid nursing home placement, or avoid re-hospitalization,” says Jane Hanahan, RN, BSN, MHA, administrative director, CKHS Home Care and Hospice.

 

CKHS home care teams achieved significantly lower rates of re-hospitalization than the national average of 29.8 percent.  DCMH achieved 19.9 percent; Taylor, 14.8 percent; and Crozer-Chester Medical Center, 19.9 percent. Factors driving their success include a patient education tool, developed by a nursing work group, which directs the patient when to call a nurse or physician and when to call 911. In addition, nurses were educated on the SBAR (Situation, Background, Assessment, Recommendation) method of reporting to physicians, and the home care teams changed their practice to “front load” visits during the period when patients are most at risk for re-admission.

          

The HomeCare Elite, the only performance recognition of its kind, is compiled by OCS, Inc., a provider of health care rating information, and DecisionHealth, publisher of the home care newsletter, Home Health Line.
