The Goal of Crozer-Keystone Health System’s Community Health Education Program is to improve the overall health status of residents in Delaware County. Through assessing community needs and assets CKHS produces a biannual Community Health Needs Assessment. Community Health Education uses this tool to measure the progress of key health status indicators and reports trends over time in order to target risk reduction and health promotion intervention initiatives for maximum effectiveness.
Based on the national Healthy People 2010 objectives, which are set by the Centers for Disease Control and Prevention, Crozer-Keystone has identified major areas of focus in Community Health over the next 10 years:

• Maternal, Infant, and Child Health
• Promotion of Responsible Sexual Behavior
• Cardiovascular Health Improvement
• Early Detection of Cancer
• Improvement of Behavioral Health
Improving the health of our community is profoundly affected by the collective beliefs, attitudes and behaviors of everyone who lives in the community. Crozer-Keystone Community Health Education shares this responsibility with both its internal and community partners as they strive to improve the health status of the individuals in Delaware County and the surrounding area.
