The ACE Unit is designed to be safe, comfortable and easy for seniors to use. This approach starts with a physical environment filled with design features and amenities to ensure the safety and convenience of our patients.

The Crozer ACE team works with each patient’s attending physician to design and provide personalized treatment. The attending physician will continue to oversee your plan of care with assistance from the ACE team.

Our personalized approach to treatment includes an assessment of the patient’s current physical, mental and emotional well-being. We develop a plan to help each patient maintain or regain the functional abilities that he or she enjoyed before hospitalization.

The ACE team regularly “rounds” together, visiting each patient to assess his or her current condition. The ACE team also makes plans so that the patient’s health care and daily living needs are met after he or she is discharged from the medical center.
